Full job description

The management of delayed discharges.

The management of Complex discharges and Case management of the Complex cases.

To promote safe, effective discharge planning to allow patients of the Trust an informed supported transfer of care to a suitable setting e.g. home, home with support services, rehabilitation facilities, intermediate care, residential or nursing home.

To monitor/act upon delayed discharges.

To ensure effective use of discharge lounge, managing staff and activity within the department. Confirming District nurse referrals, Package ages of care, Cut off times for Care homes

Provide direct patient care for patients within the Lounge, this includes medication round, personal hygiene and moving and handling.

To support the Clinical and non-clinical Management in overseeing all activities within the Discharge hub, deputising in the absence as directed.

Responsibilities:
Management

Acting as an expert for advice and support for clinical areas and the organisation in a planned and managed process.

To attend bed meetings.

To attend medical hand back meeting and liaise with consultants regarding discharge planning.

To provide line management responsibility to allocated Patient flow coordinators.

To lead on management of all Early Discharge Notification & Need based assessments in the hospital and the process and selection of appropriate referral.

Attend the HHCP regarding discharge planning on weekends and when required.

To undertake special projects, as required, which develop the understanding and knowledge of discharge planning.

To liaise with community Trust and ICB’s to ensure efficient utilisation of community beds, co-ordinating the transfer of patients for rehabilitation.

To agree a Personal Development Plan following an annual performance review with the Senior Discharge Co-ordinator that meets individual and organisational needs.

To act as a nominated deputy fire officer in conjunction with the Clinical Site Practitioner.

To share information with Site Practitioner regarding potential discharge/transfers.

To ensure good working relationships between the Trust and transport services.

To ensure good working relationships between the Site Practitioner Team and other departments, both internally and externally.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.
